In regards to parental consent for testing for COVID-19 in schools a parent/guardian may provide consent using the form found at this link. https://www.schools.nyc.gov/school-life/health-and-wellness/covid-information/covid-19-testing-for-students or a parent/guardian can use the New York City School Account (NYCSA) at schoolsaccount.nyc.
